ABSTRACT

Objective To investigate the effect of bilateral knee osteoarthritis (KOA) ondynamic balance ability of ankle strategy in aged women. Methods The dynamic balance ability tester was used to test the balance score, the rotation speed, the maximum rotation speed, and the percentage of the target ball's residence time in each area of KOA patients (KOA group)and the general elderly (control group), and a comparative analysis between groups was conducted. Results The balance score of KOA group was lower than that of control group; the dynamic balance control ability of KOA group in the horizontal direction was basically the same as that of control group, but the dynamic balance control ability of KOA group in the vertical direction was weaker than that of control group.Bilateral KOA reduced dynamic balance ability of ankle strategy in the aged women. It could not affect the left-right symmetric balance ability of the aged women, but it would reduce its forward-backward symmetrical balance ability. Conclusions Bilateral KOA aged women may be more likely to fall forward or backward, while not easy to fall laterally. For elderly female patients with bilateral KOA, methods such as strengthening ankle joint strength, proprioception and responsiveness can be used to prevent falls that may be caused by reduced dynamic balance ability, especially falls in the forward and backward directions.

ABSTRACT

The chemotherapy combined with photothermal therapy has been a favorable approach for the treatment of breast cancer. In present study, nanoparticles with the characteristics of photothermal/matrix metalloproteinase-2 (MMP-2) dual-responsive, tumor targeting, and size-variability were designed for enhancing the antitumor efficacy and achieving "on-demand" drug release markedly. Based on the thermal sensitivity of gelatin, we designed a size-variable gelatin nanoparticle (GNP) to encapsulate indocyanine green (ICG) and doxorubicin (DOX). Under an 808 nm laser irradiation, GNP-DOX/ICG responded photothermally and swelled in size from 71.58 ± 4.28 to 160.80 ± 9.51 nm, which was beneficial for particle retention in the tumor sites and release of the loaded therapeutics. Additionally, GNP-DOX/ICG showed a size reduction of the particles to 33.24 ± 4.11 nm and further improved drug release with the degradation of overexpressed MMP-2 in tumor. In the subsequently performed

ABSTRACT

Objective To construct an early warning model of fall risk for the elderly based on six kinds gait parameters. Methods A digital field was used to collect parameters from six kinds of gait for the elderly with or without the history of falls, and the binomial logistic regression analysis was used to establish a regression equation for predicting the fall risks in the elderly, and an early warning model was constructed. Results The regression equations constructed according to the parameters from six kinds of gait were statistically significant. The overall correct rate was predicted from high to low: walking forward with closed eyes (97.1%), walking backward with open eyes (92.9%), walking backward with closed eyes (88.6%), walking forward with open eyes (87.1%), turning head up and down with open eyes (85.7%), turning head left and right with open eyes(82.9%). The constructed early warning model for fall risk of the elderly mainly included five steps, namely, judgment, test, extraction, calculation and early warning, which was suitable for gait testing and evaluation of the elderly in the laboratory. Conclusions Parameters from six kinds of gait could predict the fall risk of the elderly. Among them, walking forward with closed eyes was best to predict the fall risk in the elderly. The established early warning model of fall risk for the elderly could be used to predict the fall risk of 65-75 year old people within one year, which could provide early warning based on the probability of falling, playing a positive effect on preventing falls in the elderly.

ABSTRACT

Objective@#To analyze the prognostic value of CD7 expression in newly diagnosed acute myeloid leukemia (AML) patients, and to further explore the correlation between CD7 expression and CEBPA mutation, and to clarify the prognostic value of CD7+ in AML patients with wild-type (WT) or mutant-type (MT) CEBPA.@*Methods@#The clinical data of 298 newly diagnosed non-M3 AML patients between January 2010 and December 2016 were analyzed retrospectively. The clinical characteristics and prognosis of CD7+ and CD7- patients were respectively compared in all patients, and in patients with WT and MT CEBPA. The relationship between CD7 expression and CEBPA mutation was determined by chi-square, and the effects of CEBPA mutation on survival and prognosis in CD7+ group by Kaplan-Meier method.@*Results@#In CD7+ group, the frequencies of CEBPA mutation were 10.1% (single site) and 33.9% (double site) , significantly higher than those of the CD7- group (5.3% and 4.2%) (P=0.000) . Subgroup prognostic analysis showed a lower CR rate (P=0.001) and a higher RR (P=0.023) in CD7+ group comparing to those of CD7- group in AML patients with wild type CEBPA. There were no statistical difference between CD7+ group and CD7- group in overall survival (OS) and disease free survival (P>0.05) , while in the CEBPA mutant group the CD7+ group has higher OS (P=0.019) and DFS (P=0.010) . Based on the CD7 expression and CEBPA mutation, 298 cases were divided into 3 subgroups, named as CD7+-CEBPA MT group, CD7- and CD7+-CEBPA WT group. The 3-year OS of the 3 groups were 80.2%, 48.0% and 30.6%, respectively (P<0.001) , and the 3-year DFS were 74.1%, 37.4% and 22.2%, respectively (P<0.001) .@*Conclusion@#The CEBPA mutation rate was higher in CD7+ AML patients then that of CD7- patients. CD7 expression has opposite prognostic significance in AML patients carrying the wild-type or mutant-type CEBPA. Based on CD7 expression and CEBPA mutation, a new risk stratification model can be established, which is helpful to guide the clinical individualized treatment for AML patients.

ABSTRACT

OBJECTIVE To invest igate the therapeutic effect and mechanism of PPAR gamma agonist on allergic rhinitis(AR) in mice. METHODS AR murine model was established by OVA sensitization and challenge. The behavior observation was used to understand the improvement effect of PIO on AR symptoms. The morphological characteristics of nasal tissues were observed by HE staining. The total RNA was extracted to investigate the level of mRNA expression of Foxp3, T-bet and GATA-3. The changes of CD4+Foxp3+T cells in spleen of mice were analyzed by flow cytometry. RESULTS BALB/c mice received OVA sensitization followed by OVA intranasal challenge, the frequencies of sneezing and nose-scratching increased signif icantly in AR group compared with control group. The frequencies decreased significantly in PIO group, compared with AR group. The continuity of nasal mucosa ciliated columnar epithelium in AR group was destroyed and appeared to be repaired in PIO group. Inflammatory cells infiltration was also markedly decreased by PIO treatment. PIO significantly increased the expression of Foxp3 mRNA(P <0.001) compared with AR and control group. There was no significant difference in T-bet between PIO group and AR group, but the expression of GATA-3 mRA in PIO group was significantly lower than AR group. The proportion of CD4+Foxp3+T cells in AR group (4.43%±0.25%) decreased compared with control group (5.19%±0.39%) (P <0.001). PIO treatment induced production of Tregs (6.35%±0.37%) compaered with control group(P <0.001). CONCLUSION PPAR-gamma agonist can effectively alleviate allergic symptoms of mice and regulate the balance of Th1/Th2. The role of PPAR gamma agonist in the treatment of AR may be the amplification of Tregs by promoting Foxp3 expression.

ABSTRACT

@#To investigate the hypoglycemic effects of terpenes from Fructus Corni(TFC)on type 2 diabetes mellitus, the db/db diabetic mice were intragastrically administered with 25, 50, 100 mg/kg of TFC for 10 weeks. The fasting blood glucose, insulin(Ins), glycosylated serum protein(GSP), total cholesterol(TC)and triglyceride(TG)levels were determined. At weeks 8 and 10, intraperitoneal injections of glucose and gavage starch tolerance tests were performed, respectively. The db/db mice showed obvious obesity. Each dose of TFC could significantly reduce the body weight of db/db mice(P< 0. 05). After 4 weeks of administration, all doses of TFC significantly reduced the fasting blood glucose of db/db mice(P< 0. 05). The serum TC, TG levels were also significantly decreased in the TFC middle- and high-dose groups(P< 0. 05). In addition, middle- and high-dose of TFC could significantly reduce the level of GSP. Middle- and high-dose of TFC also significantly improve the glucose tolerance and gavage starch tolerance in db/db mice(P< 0. 05). These results suggest that TFC could improve diabetes-related symptoms via regulating glucose and lipids metabolism.

ABSTRACT

This paper uses the data collected from 138,477 patient discharge records of 37 Shanghai tertiary public hospitals in December 2013 and classifies all the records into different groups by using a localized diagnosis re-lated group tool. The coefficient of variation index ( CV) of each group was used to analyze the dispersion of average hospital cost per inpatient day for each group, in order to evaluate the suitability of diagnosis related group systems and to set the coefficients of difficulty ( relative weights) for each disease. The study has found that severity-based di-agnosis related group systems are more suitable and consistent. However, future studies should collect more detailed information on health costs in order to reflect the value of health professionals and technologies and therefore further improve diagnosis related classification and develop new systems that can be adapted to local conditions by applying the weights of different groups.

ABSTRACT

Objective To investigate the clinical pathological characteristics, diagnosis and treatment of breast rhabdomyosarcoma, and to enhance the awareness of malignancy infiltration to bone marrow (BM). Methods The data of one case of Rhabdomyosarcoma of breast were analyzed retrospectively. BM aspirate and biopsy, morphology, immunology, cytogenetics, molecular biology (MICM) in different parts of BM, peripheral blood smear, fine puncture of breast mass, final biopsy of breast mass by Mammotome System and whole body PET-CT were performed. The immunochemistry stain of specimen of breast mass was used. Results The peripheral blood smear of this patient showed immature erythrocytes, leucocytes and classification of unknown cells which were consistent with BM morphology. The results of BM aspirate and biopsy depicted a hypercellular specimen with disseminated unknown cells infiltration. Unknown cells were positive for CD56 and negative for any hematopoietic markers by flow cytometry. The whole body PET-CT showed that uptake of 18F-FDG of bilateral breast and whole BM was increased, whereas the mass of breast was not presented by CT. PET-CT suggested a probable malignant hematologic disease. The enough specimen of breast mass got from Mammotome System showed embryonal rhabdomyosarcoma, and the tumor cells were positive for MyoD1, Vimentin and Desmin. Conclusions It is a challenge for early diagnosis of solid sarcoma with unknown origin which diffusely infiltrating into BM. Negative expression of hematopoietic markers by flow cytometry plays a role on differential diagnosis in this setting, whereas PET-CT only provides a valuable reference. Enough specimen and immunohistochemical staining could provide solid evidences of diagnosis.

ABSTRACT

Polymyxin E shows effective treatment of the infection induced by resistant gramnegative bacteria, but its nephrotoxicity severely limits the clinical application of this drug. In this work, methoxypolyethylene glycols 2000 (mPEG2K)-polymyxin E (PME) was synthesized via chemical grafting reaction and had been characterized. The antimicrobial activity and cytotoxicity of mPEG2K-PME in vitro were investigated on Escherichia coli and HK-2 cells, separately. Intra-abdominal infection model was further established in order to study the therapeutic effect and the toxic effect on kidney of mice. The results showed that mPEG2K-PME exhibited significant inhibitory effect on Escherichia coli and had a lower toxicity on HK-2 cells in vitro. At the same time, mPEG2K-PME had a good efficacy in the treatment of Escherichia coli infected mice in vivo. Moreover, nephrotoxicity caused by mPEG2K-PME was significantly reduced compared to free PME. mPEG2K-PME is promising in development of new preparations with high efficiency and low toxicity.

ABSTRACT

OBJECTIVETo observe the short- term effects of hemogram in donors after peripheral blood stem cell(PBSC)collection and donors' tolerance.

METHODSA total of 166 related allogeneic donors were selected from The First Affiliated Hospital of Medical School of Zhejiang University between January 2013 and December 2014, including 86 male and 80 female. All donors accepted granulocytecolony- stimulating factor(G-CSF)5-10 μg·kg⁻¹·d⁻¹ until collection finished and were measured by blood cells count before and after PBSC collection.

RESULTSAfter PBSC collection, the hemoglobin level decreased from 145(94-181)g/L to 138(93-167)g/L, and the platelet counts decreased in all donors from 231(105- 490)× 10⁹/L to 95(39- 210)× 10⁹/L. The amount of hemoglobin contamination in collection products was weak correlated with the decreased hemoglobin in peripheral blood(r=0.297, P=0.017), and the platelet contamination was high correlated with that decreased in peripheral blood(r=0.719, P<0.001). The decline of hemoglobin level after twice PBSC collection was of no significant difference between four groups in different ages(P≥0.05). The decline of platelet counts was out of a significant difference(P> 0.05). In addition, the decline of hemoglobin level after once and twice PBSC collection was of a significant difference between four groups in different body mass index(BMI)(P=0.003 and P<0.001), especially in thinner group with obvious decrease. But the decline of platelet counts was out of a significant difference (P>0.05).

CONCLUSIONThe hemoglobin level decreased mildly in healthy allogeneic hematopoietic stem cell donors after PBSC collection and it is better to adjust parameters every time to ensure their safety for thinner donors. However, it will increase the risk of platelet decline, which is unrelated with ages and BMI and can be tolerated.

ABSTRACT

Aim To optimize the most effective compo-nent formula from the active ingredients of Salvia Milti-orrhiza and Panax Ginseng through the orthogonal de-sign method to resist breast cancer, and to reveal its antitumor mechanism in MCF-7 cells. Methods The human breast cancer cells MCF-7 were employed as the research object and the normal breast epithelial cells MCF-10A were used as control, optimizing the most ef-fective component formula from the active ingredients of Salvia Miltiorrhiza and Panax Ginseng by using CCK-8 assay and orthogonal design method; real-time cell a-nalysis was used to monitor the best combination formu-la on cell proliferation, and high content screening was used to detect the best combination drug on cell apop-tosis. Results The best combination of the salvianolic acids, saponins of Panax Ginseng and ginseng polysac-charides that were screened out were 5 , 10 , 5 mg · L-1 . Compared with control group, the treatment group had effective response inhibiting the proliferation on MCF-7 cells, but those effects were weaker on MCF-10A cells through real-time cell analysis. Ho-echst, Annexin V, PI staining fluorescence showed no significant difference ( P >0. 05 ) on MCF-10 A cells compared with the control group, but there was signifi-cant difference ( P <0. 01 ) on MCF-7 cells by HCS. Conclusions The most effective component formula from the active ingredients of Salvia Miltiorrhiza and Panax Ginseng have a strong inhibition of proliferation and induction of apoptosis to resist breast cancer with selection, and there is no significant difference in MCF-10A cells.

ABSTRACT

OBJECTIVETo study the expressions of Notch1-4 gene in human keloid-derived mesenchymal-like stem cells, and to explore the Notch signaling pathway's role in the formation of keloid.

METHODSKeloid samples were collected to harvest human keloid-derived mesenchymal-like stem cells through two-step enzymatic dissociation method. By flow cytometry, cell phenotype of primary and P3 generation were analyzed. By immunocytochemistry, the expressions of Oct4, vimentin and CK19 were examined. Keloid-derived mesenchymal-like stem cells were induced into osteoblasts in vitro and calcium deposition was detected by Alizarin red S stain. Realtime polymerase chain reaction (RT-PCR) was used to detect the expressions of Notch1-4 mRNA in keloid-derived mesenchymal-like stem cells.

RESULTSFlow cytometry showed that keloid-derived mesenchymal-like stem cells of primary and P3 generation highly expressed CD29, CD44, CD90 from the typical MSC phenotype marker, but they failed to express HSC phenotype markers, such as CD34 and CD45. The results of immunocytochemistry showed that Oct4 from pluripotent stem cell markers and vimentin from mesenchymal cell markers was positive and CK19 from epithelial cell markers was negative. After induced differentiation into osteoblasts in vitro after 21 day, calcium nodules could be seen clearly; Notch1-4 gene were expressed in keloid-derived mesenchymal-like stem cells through RT-PCR. The relative quantitative of Notch2, Notch3 gene were higher than Notch1, Notch4 gene (P < 0.05).

CONCLUSIONSThe expression difference of different subtypes from Notch gene in human keloid-derived mesenchymal-like stem ceils may be related to self-renewal, proliferation, differentiation, and participate in the formation of keloid.

ABSTRACT

ObjectiveTo develop a quantitative method for simultaneously determining multi-components in Rhei Radix et Rhizoma using one chemical reference substance.MethodsThe contents of multi-components were calculated by the UV relative correction factors (RCFs) of chrysophanol, physcion, and rhein to emodin.ResultsThe values of RCFs at 274 nm for rhein, chrysophanol, and physcion to emodin were 0.712,0.674,and 1.051.The calibration curves were linear over the ranges of 0.02-4.08,0.02-4.12,0.07- 12.92,and 0.02-3.68 μg/mL for rhein, emodin, chrysophanol, and physcion, respectively. The contents of emodin in 18 samples were determined by the extemal standard method, and the contents of the other three anthraquinone aglycones were calculated according to their RCFs.ConclusionNo significant difference is found in comparison with the classical method, indicating that the RCFs have high reliability within their linear ranges and could be used in quality control of Rhei Radix et Rhizoma. The quantitative analysis of multi-component with a single marker is especially suitable for herbal medicines containing unstable or hard to be purified components as quality control markers.
